Thank you for scheduling a counseling or coaching appointment with a member of our team. We are grateful for the chance to come alongside to serve you and those you care about. Now that you have called for an appointment, please take time to complete the intake forms to bring with you to your first visit. This will save considerable time, (and money), by allowing our counselors to rapidly assess your needs and begin to quickly guide you on the journey of building stability and strength.
If you have any questions about completing these confidential forms, please notate them to discuss with your counselor in person as you begin to move forward toward greater results by taking positive action to improve your life. LifeWorks Counseling Registration
This confidential form is needed for every counseling client visiting our office. If you are completing this form for someone other than yourself, (such as for a child under the age of 18), complete the answers as it might help the counselor to understand their needs.
LifeWorks Counseling Life History
This confidential form is extremely helpful for your counselor to have in understanding the major events and experiences in your life and your current work and relationship status.
LifeWorks Counseling consent form, (for minors)
This confidential form is only needed when a minor child will be seen by a LifeWorks Group counselor as part of individual or family counseling.

This form is for you to read and review to understand the many steps we take to protect your confidentiality; as well as to understand the administrative procedures we take to serve you most effectively.
